Membership Terms

• $50/month with a 6-month minimum commitment.

• $25 sign-on bonus will be applied after your first monthly payment is processed.

• Early termination fee: If you cancel before the 6-month commitment is fulfilled, a $50 fee will be charged for each remaining month.

• Account info, including a valid credit card, must remain current in your patient profile.

• Cancellation must be submitted prior to your next billing cycle.

• Membership benefits apply only during active membership.

• Memberships are non-transferable and non-shareable.
